Where do I get forms to file for a divorce, name change, change of custody, etc? What are filing fees?

We do not provide any forms for pro se filings of any type. We cannot give legal advice. We will accept pro se filings but you must have the documents prepared when you come to our office to file the documents.


You can buy packets at office supply stores or download them from the internet. Filing fees are $209.00 plus $50.00 sheriff’s service fee if applicable. We do not accept credit or debit cards for payment.
