This Building Permit Application MASTER CONTRACTOR SHEET must accompany the Private Dwelling Permit Application. This form is to be completed by the General Contractor, Electrician, Plumber, and Heating & Air Contractors. This form must be completed, signed and submitted before a permit is issued or work is commenced. A copy of the current Business Licenses and State Certification Cards must also be on file with the Permit Office. A new "updated" original must be obtained each year. The Permit Office will verify, sign and date the original and returned it to the General Contractor. A copy of the verified original may be submitted for the next permit. The General Contractor is responsible for this form.
