Who Can Obtain A Certified Copy Of A Death Certificate Containing The Cause of Death 

• Spouse of deceased
• Either parent of the deceased
• Adult  child of the deceased
• Adult brother/sister of the deceased
• Legal representative of the estate of the deceased

A certified copy of a death certificate, which does not contain the cause of death or a social security number, may be issued to anyone upon completion of an application.  Costs for these records are as follows:  First copy $25.00 and each additional copy $5.00 (if issued at the same time for the same person).  Payment is accepted by cash or money order only.  Checks and credit cards are not accepted.
